Is the the text: http://www.michigan.gov/documents/Re...A_132931_7.pdf? It sounds like as long as you follow the directions in the first couple pages you can print it out yourself (if you have access to a printer that can handle the proper size). If not, I'd take it to Kinko's or Staples or something. I wouldn't be surprised if one of those places had one already set up and ready to go actually. We had one on the wall at the TV shop I used to work at, gave the rates:
Labor: $15/hr If you watch: $20/hr If you help: $25/hr If you tried to fix it and couldn't: $50/hr You can update the rates to reflect 2011 instead of 1979.
