Default Driveline Vibrations on New Lifted Ram

OK, I was doing some reading of past posts here on Vibrations.

On my 1992 Ramcharger with a new 4" Softride FULL(no blocks) suspension lift It seems like I have a Rear Shaft Vibration from 55MPH through 75MPH. My degree variation from the T-Case to the Pinion is Over 4-5 Degrees. I have NO shims on the truck at all. What I'm looking at is doing a CV Joint in the Rear. I noticed that someone in a recent post took the Front flange from the T-case and put it on the back, then used a lengthened front shaft on the rear.....Here are my questions:

Does anyone have expierence doing this?
What T-Case do I have?
Part Number for the Front Flange?

the tcase is a 241. im gussing the drive shaft is close to being maxed out with 4" of lift and thats where the vibration is comming from. i dont recall seeing anyone using a cardone joint in the rear with 4" of lift.
