OH, Ive sort of had another, not really an accident, but weird none the less... We used to have an old Stevens side by side 12ga with a single trigger. It developed a habit of discharging both barrels on a single pull of the trigger. The gunsmith said that was common with that model, and it wasn't worth getting fixed.

I had that happen to while trying to teach my ex-girlfriend how to set the safety. Needless to say she got a whole lot of respect real quick for that gun. I never had to tell her how to use it after that.

Shot a hole through the dining room ceiling with a 22 when I was a kid while my parents were at the neighbors.
Filled the hole with Collgate tooth paste and never said a word.

Had one with a "high powered" pellet gun. had the breach(SP) style cocking went to close it and it just shot. Dad did 2 more dry fires and it did the same trigger stuck.

While hunting elk in Colorado, my dads friend was unloading his 7mm mag ( this guy was anal about safety and keeping the gun pointing in safe direction) as he racked the 2nd or 3rd shell out, the gun fired. Luckily it was pointed up and out of harms way as we all were standing around the truck chatting.
This gave me a whole new respect for guns.

In Korea, while relieving the guard at an underground magazine, the sentry used the clearing barrel while removing his magazine and then firing into the barrel. He discharged a round and scared the shit outta both of us; everyone expects it to be a dry fire, and the one time it isn't, wow! So after reporting the AD to our SSgt, we had to sift through the clearing barrel sand and find the projectile in order to prove the AD and to have inventory accountability. We doubt the inventory thing, we think he just wanted to 'teach us a lesson'.

didnt happen to me, but the cop comment on the last page reminded me of one of our cops in clare county. he was playin around in the jail, doin the quickdraw thing, and shot himself in the leg! needless to say, hes walkin on half a wooden leg now.

Yes.

Deer hunting with a borrowed Remington 870 slug gun.

Walking in a corn field, got a corn stalk up the leg of my coveralls, tripped me, and while keeping grip on gun, removed safety and pulled trigger. It's hard to explain, but I have a habit of cradling my hand over the trigger to protect it from brush, etc... when walking through overgown stuff looking for critters. My glove got tangled in the trigger, and it was pulled.

Thankfully, Dad drilled safe gun handling into my skull from an early age, so while I got caught up in the trigger and fell, the gun was kept pointed at the ground and all I did was waste a copper solid sabot slug and embarass myself. Gave me even more religion on safe gun handling.

Dad had one as well. After having a stroke and losing some dexterity, he ended up dropping the hammer on his 1911 while unloading it (loss of hand strength and coordination issues) and because he was practicing thoughtful gun handling by keeping the muzzle pointed in a safe direction, he dispatched his new Serta Perfect Sleeper with a .45 silvertip.

Guess who Dad gave his 1911 to that night?

i did have 2 chambers fire at the same time on a .44 black powder pistol once.
it left a nice lead stripe down the side of the frame and barrel. luckily black powder pistols dont have the recoil of a .44 mag. both hit the target.

i am always very careful dropping the hammer on my 1911 with one in the chamber.
i can see how it could happen.

I was dropping the hammer on my .45 in the bedroom one evening before bed and thumb slipped off the hammer and my mattress caught the slug, i no longer hammer de-cock without dropping the mag and clearing the chamber first...
