2000 Chevy 2500 hard shifts

I just picked up a 2000 Chevy 2500 with 6.0 and 4 spd auto. After driving on the highway for a while, the trans upshifts really hard after you come to a stop at the end of an off ramp and get back up to speed. It feels like the torque converter isn't unlocking for the shifts after it's been locked up in 4th for a while on the highway. Turning the truck off and back on did not result in any softer shifts. After sitting for a few hours, it was back to normal.

Any thoughts on what might be causing this? Do I need a new torque converter? Or could it be as simple as a sensor somewhere? I believe the fluid has been changed in the last 50k miles. Truck has 111k on the clock and I am the second owner.
