Can anyone identify this "trinket"

So, I have had this thing for almost 20 years and have never known what it was. I found it in a hotel lobby inside a desk drawer. I was in grade school at the time, playing in a basketball tournament in Grand Rapids.

It is made out of some kind of metal.
It has a part that slides out as seen in the picture

Please help me cure my curiosity

I'm going to send pics to my cousin, he runs an antique shop in Denver, and he collects antique tie tacs and cuff links. If anybody would know for sure, it would be him.

I had someone tell me they thought it was a drug users tool? Roach clips with a bowl cleaner on the pull out part. The bony back on the fish would be used to break up rock cocaine. Interesting
