I partially did the swap...
By partially, I mean, my cab was from an auto, but my frame and running gear are from a manual, a first gen at that.
First, you need to swap in the trans itself. You'd need a flywheel/clutch to replace your torque converter/pressure plate. I don't know about the crossmember. I can possibly make a measurement this weekend, I think they are quite close, but not 100% sure if they are exactly in the same spot. My old auto is in the bed of my truck, ready to go to the scrapper..
Pull the old shifter, trans computer, and cut the holes where your trans and t-case will need them. I used a jigsaw to engarge the original hole, and used a 4" hole saw to cut the hole for my 2nd t-case. I use a short trans, though(L52) so your locations might be different.
Once you get it installed, next you need the clutch pedal.
First, you need to drill holes for the master cyl. All of this is a lot easier if you remove the dash, bit can be done with it in place(I left mine in place). Theres a bog hole for the bore of the master cyl, and two holes for the bolts/studs. I used a right angle drill to drill some small holes from under the dash, then using the smalle rholes at centers, I enlarged them from inside the engine compartment. Use a die grinder/dremel to clean them to a nice fit. Have a friend help you as its tricky to get the master cyl bolted in place by yourself(although I managed it ok..)
The bracket is already in place, you just need to take the pedal and install it into the bracket(just a pin with a cotter pin to hold it in place, and a screw to adjust the pedal stop. You might need to replace or cut the brake pedal to be smaller.
Hook up your slave cyl, should pretty much bolt in place.
Now, look at the wiring that went down to the automatic. There were a couple different connectors. One of them, a decent size round plug, has just three wires, two of which are large and black, about 12 gage or so. On the vehicle side, cut those two big black wires and solder them together. That is the neutral safety switch. The truck won't start without those jumped.
That should be all you need.
